Since there seems to have been a doctor involved and a post mortem was deemed unnecessary it is exremely unlikely that there was an inquest. The decision to involve the coroner would be taken by the registrar before the death certificate was issued

She died of
(1) childbirth (7 hours) and (2) antepartum haemorrhage (accidental)
which means she was haemorrhaging (bleeding) before the birth. This may well have resulted from a placental abruption, where the placenta detaches from the wall of the uterus, causing bleeding which may be catastrophic.
If there is a high degree of separation of the placenta it is an emergency both for the expectant mother and her unborn baby, requiring immediate delivery even if the baby is very premature. If there is substantial blood loss both mother and baby are at risk of death.
It looks as though the cause of death was obvious and certified by the physician so, as others have said, there would have been no need for an inquest (hence the annotation "No P[ost] M[ortem]").

If there had been an Inquest, the informant on the death certificate would have been the Coroner.

Anna are/were you a midwife?
I used to be a midwife and have only seen a very few placental abruptions, very easy to diagnose because of the pain the woman is feeling, and caesarean sections are carried out immediately. They are normally before labour commences and Brian T's gran had already been in labour for 7 hours. I was going to say it wouldn't happen today, but was listening to Woman's Hour this morning about stillbirths and was horrified at what goes on in maternity units nowadays.

Oh, Liz, I certainly don't purport to be an expert on the subject and I didn't mean to sound as though I was holding myself out as such. I'm not a midwife, I'm a barrister, and in my line of work (litigation) we sadly see the aftermath of catastrophic events when they have gone badly wrong and someone is suing someone else. I suppose it follows that what I see in court cases tends to be the more serious end of the spectrum.
I have seen cases when I have been doing volunteer aid work in Africa (not in any professional capacity) where abruptions have happened and for one reason or another there isn't the opportunity of a caesarean, and the mother has endured many hours. Sometimes they started labour normally but problems developed later on due to a low-lying placenta.
I was going to say it wouldn't happen today, but was listening to Woman's Hour this morning about stillbirths and was horrified at what goes on in maternity units nowadays.
I know, terrifying isn't it? One wonders whether after years of progress things are going backwards now.
