The registers of the Bishops of Hereford Diocese from 1275 to 1900 have been transcribed and published at:

http://www.melocki.org.uk/MelockiDiocese.html

(new website wef 1 August 2009)

The transcripts list all institutions to every Hereford benefice in the period and include the date, and names of the presentee (priest appointed), patron, and previous occupant. They are available as web pages indexed alphabetically by benefice (aka parish in most cases), and also as spreadsheets.
