Windows updates are the necessary?

I have Windows Vista and Kapersky Internet Security installed on my computer, Nearly every day I get at least one notification sometimes more of Windows updates listed as important ie Windows Defender, security updates, firewall updates etc etc, not being well up with this computer business it seems as if all these details some of which are fairly large downloads would be taken care of by my Kapersky system which is set to automatically update, so the question is do I need to take up these Windows updates.

I would always update windows security rather than rely on a secondary program like Kapersky.  FYI I hate Kapersky and find it a waste of money but others will disagree.  Windows is well known for having security issues and some of these Kapersky nor any other AV program can protect you against.  Items such as buffer overloads etc but that is out of the realms of anyone with little knowledge about computers

Strictly speaking you are not referring specifically to Windows updates.

Although Microsoft supplies Windows Defender - it is not part of Windows and therefore it is not a Windows update.  As Defender is an anti-spyware program it needs to be updated at least once a day in order to be current and therefore effective.

I'm not sure which brand of firewall you are using but that does not need to be updated on a daily basis.

Windows (proper) updates should be done whenever they become available.  Some are security updates and others are simply updates (possibly optional).  You should always install the security updates - although you anti-virus program probably offers you the same or better level of protection in terms of Window's MRT (Malicious Removal Tool).

Some years ago my doctor told me to take a few days off work to get over a bug I'd picked up. We were especially busy so I asked him if it were really necessary.

His reply was alongs the lines of not strictly necessary but definitely advisable. I think that advice applies to Windows updates as well.

Kaspersky isn't bad, but ESET Smart Security is better - it doesn't slow your computer down anywhere near as much.  As you said, most Windows updates are security patches, so you should always download them.
