Nice one Zeb, I've had complaints that I haven't put an email address on my site so nobody can contact me, reason being reluctance to give out the email address and this gets around it nicely.
But .... and I'm sure I won't be the only dummy asking this question so I might as well be the first - not having used php before, how do I call it from a html page please? If this is covered elsewhere could maybe Bob just post the link as a reply and I'll read up from there.
Thanks for your effort Zeb, I'm sure lots of people will appreciate it.

Added after thinking about it
duh!!! You just put a link as normal right?? instead of linking to .html you link it to the emailme.php. Guess I need another coffee and wake up. 
I still think it's a great script, thanks

Link to it using something like this:
<a href="emailme.php">Contact Me</a> I prefer to have forms like this beause as yuo say, you don't have to have your email address made public. I know people who write scripts to trawl through websites digging out information. This can be abused by pulling out email addresses from sites to use for spamming. Viewing the source of the script won't show the email address because its embedded in the PHP section.
When posting my email address publically I add "NOSPAM" after the @ like this: ivan@NOSPAMzeboliver.co.uk. Any bots then pick up the entire email and of course, with NOSPAM in it, its invalid. Another way is writing it ivanATzeboliverDOTcoDOTuk

I used to insert such forms into my early websites, but I abandoned the device ages ago. There are two basic problems:
- Anonymous pranksters can send you rubbish.
- Genuine visitors can make an error in their email address, and you never know who contacted you.
Nothing beats email. Well yes, there's something that we all know, which is almost as good as email, at least for members of this forum: instant messages.
Another nasty thing. If an organization gives out free webspace and PHP/database services to members, combined with the possibility of sending out emails, this might well be (one never knows) an open invitation to hackers to use this organization's server for sending out spam. Here in France, the Free organization, which sponsors most of my websites, does not allow members to use their websites for emailing, and that constraint doesn't bother me at all.

There is probably no "best solution", everyone has to decide for themselves.

Okey dokes I have managed to edit and upload a form... when i go to send a test message tho, the form goes blank. I had a look through the script but couldn't see a 'go to here' url... or am I a bit blind? (maybe more coffee needed lol). I haven't yet linked it to another page - does that make a difference?

Hmmmm.... okay my form action section looks like the second one i.e. <form action="emailme.php" method="post"> and i've linked it to the index page... found i had messed up the script (wow what a surprise hehe). Woohoo! Success - message arrived to my inbox 
Also - I would like to remove the drop down for 'surname' but since my last doctor-job went a little bung I will ask this time before diving in
