I have found my father's attestation on Find My Past.

I have attached a copy.
Is anyone able to explain the note written below his army number?

What do the two numbers below his name mean?

The group below the surname is a National Registration number.  These were introduced as part of the system which produced Identity Cards for civilians, late in 1939, and they continued to be used for National Health purposes for many years.  The /3 usually meant that he was the third member of his family to be listed.  The letters signified the town or city etc.
I cannot help with the remainder.
