Early C17th Petition

I've transcribed a fairly lengthy petition but there are a few words I'm not sure of - can anyone assist with suggestions?
They are underlined below - on lines 4, 9 and 12.

As an aside, does anyone know how to insert a word above a caret symbol (in Word 2010)?

I daresay there is a way, and someone will probably tell us. Meanwhile, one standard transcription convention is to show interlineated or inserted words between a pair of carets, like this ...

     ... showing ^interlineated words^ in text

If the inserted words are written in the margin ...

     ... showing ^words inserted^ [in margin] of text

Just one method.  :)
