Medical report 1843 Completed

Can anyone please decipher the word shown on this medical report. I've worked out 'he has for some time past been in an inefficient state owing to Lumbago, Chronic Rheumatism with ........? he is more or less constantly affected.
I was feeling quite smug having managed the rest but I just can't work out this word!

The bit before Chronic Rheumatism is 'and', though it's written as a + and joins onto the start of Chronic.  '...he has for some time past been in an inefficient state owing to Lumbago and Chronic Rheumatism with which he is more or less constantly affected'.

Gosh that was very quick and thank you all so much. There was nothing complicated about it in the end, just a very simple word! I was imagining it to be another ailment and just couldn't fathom it.
