I haven't seen anything online apart from the 2 census entries.  I was asking if there was anything more on the commemoration 

Commemorated the Nailsworth War Memorial as a Sergeant in the Canadian Army, died in the Great War.  He was son Tom Henry and Ellen Evans of Walkley Wood, Horsley,  his brother Raymond also fell.

(eg) Who was in charge of the commemoration and what info was available to show he had died and had served as a Sgt in the Canadian Army?

I am assuming that some proof is required before a name is placed on the War Memorial
