I wonder if it was another Elizabeth who married as she was listed as single in 1891

Very true, missed that one.

Had she had a previous child by a Mr Fox?

Looking very possible that she had children to a couple of different men without marriage.

Wonder if Alfred was a son of hers where he was on the census.

Yes, I suspect Elizabeth Ann is a red herring and the "correct" Elizabeth is the daughter of John and Emma (as per baptism found on Familysearch), indexed in the GRO index as Elizabeth SMEATHIRS, Dec qtr 1866 Wellingborough and with John & Emma, aged 4, in 1871: RG10/1500/116/22.

I would agree on that now as the Census for 1871 shows two families one with a Elizabeth A Smeathers (looks like the one who married) and another for just an Elizabeth Smeathers

John Smeathers   33
Emma Smeathers   29
Mary E Smeathers   11
Joseph Smeathers   8
William J Smeathers   6
Elizabeth Smeathers   4

In Finedon.

Marriage, Dec qtr 1885

Harry Johnson MUFF to Martha BATES or Charlotte MADDISON
Wellingborough 3b 314

Evidence from Ancestry trees, (for the 'other' couple), suggests strongly that he married Martha BATES. I can't find her after the marriage.
