Have you considered contacting Queensland BDM to ask them why there's two?   Probably best to quote both reference numbers,  but perhaps it is one certificate, and entered twice on the index because perhaps the indexer had difficulty reading the reference number.

Wow thank you!  Some scandal then (well it was then!).  I did try a Trove search for James earlier but it is a bit difficult with the name Oliver so thank you.

I have also contacted Queensland BMD re the two death certifcates

I've found the Funeral Notice for Brian OLIVER in the Courier-Mail 03 April 2001.
If you can PM me your email address I can send you a transcript as we can't put people's names up on RootsChat if they may be still alive [family].

In the same paper there is a Funeral Notice for a Jessie Olive OLIVER of Stafford [Brisbane suburb], with husband Kenneth George (Dec'd).  If you think this is relevant to your search, let me know so I can send you that too.
