COMPLETE - Destination?

Having a hard time trying to read what the destination on the border crossing form says.  I am pretty sure it starts off with "Mr. McDonald..." but I can't quite understand the rest.  Any help would be appreciated.

Mr McDonald, Cataract House, Niag[ara] Falls, N.Y.

(With "accept employment" written into the middle).

It seems to say -

Mr McDonald Cataract House Employment Niag(ara) Falls NY , with "accept" written above.

But what it means is a bit of a mystery to me, unless it is simply that his destination is Mr M's place at Cataract House where he has accepted employment at Niagara Falls.
