Help, Having Difficulty Reading Cause of Death & Address on Death Cert.

Hi all,

I am having a little difficulty in reading a couple of parts of my great grandfathers death certificate & am requiring a little assistance please!

The causes of death are given as:

I) Broncho Pneumonia
II) Severe Double ? Hernia

The address as:

96, ? Road SW17.

Can anyone tell me what the words are where I have put question marks please?

The bit of the death cert in question should hopefully be below.
