Per (Pre)-Monitor (per annum or pre) inspection  & if anything needing service found - necessary maintenance given.

Modern term - Service engineer (fitter or technician)

ie - examples :- MOT inspection mechanic or annual central heating boiler inspector/plumber

'Permanent way maintenance' - this has been confirmed to me by an ex-British Rail employee who held a track-walking ticket.

'Heavy work' - as described previously by Christine53 - remember that the 1939 Register was used for the administration of rationing.  Adding these words entitled the person to extra food rations.
