The Hague Population register 1913

Can anyone make out what it says under the date of death (penultimate column) on the second entry on this population register card from The Hague (http://denhaag.digitalestamboom.nl/(S(enihzbzmiiwwf3dv2syfb23o))/scan.aspx?book=R&id=292205)?

I can't make out the first line - maybe it's Dutch? te Foresh??

Also, if anyone can check my reading of (or help with the first one) the addresses listed in the Huizing column, I'd be grateful.

For what it's worth, I agree with the four addresses that you have identified, i.e. Graventstraat, Obrechtstraat, Tasmanstraat, and Populierstraat, but I can't figure out the first one either.
Seabridge : Staffordshire, Birmingham, Middlesex and elsewhere.

Remove the parenthesis (bracket) at the end! ;D
Then it works fine.

However, I'm confused?
The penultimate column on that link is to do with what church the person belonged to?
The column heading is "Kerkgenootschap of vereniging met godsdienstig doel" - which translates as "Church society or association with religious purpose".

Ah, got the image! It does look like Forest Gate cemetery West Ham. I'd suggest that GrB is Great Britain as it is only written against English (and one NI) placenames but that's not a key point here. Fascinating.

Still pondering the 'Porm ? ? ? ? golstr.' address. I do wonder if the initial letter is a 'P', it's different from the 'P' in Populierstrat.

I think you're right about "Gr. B" - Groot Brittanië.

Still looking at that street address.
I think it's ". . . . jolstr"?

I wonder if it's an abbreviated form of Cornelis Jolstraat [Corn. Jolstr.].
