My question is do I show her father as Edmonds-Bayliss, Edmonds Bayliss or Bayliss.

Have you found her birth registration - what surname does she have then?

Age 93 Havering Essex in 2005

Does this date of birth tie in with the entry for Maud Bishop on the 1939 register

Maud Baylis/Edmonds Bayliss birth 5 May 1912 and registered Jun 1912 Holborn. Assume then my Maud did not marry William Godfrey??

Using the d-o-b from the Maud Elizabeth Godfrey death (1 March 1912) brings up
Maude E Bishop in Hackney, with Charles ? Bishop, and 1 more person (1939 free index)

Further experimentation(?) with the free index reveals that other person to be
Kenneth Bishop, born 1936

And that apparently is the transcribed birth date for the Kenneth Bishop with Maude E and Charles ?
in Hackney in 1939 (using the free index)

There's now a baptism record on ancestry for Maud in the updated London, England, Church of England Births and Baptisms (goes up to 1916 now)

It was at St Paul Clerkenwell, but currently it is one of those that doesn't have an image available.
But the transcript could give her date of birth!
