Search for father's Royal Navy war record

Can anyone tell me what personal information would a recruit have to provide in order to join up at the beginning of the 2nd World War?
My father was adopted as a baby and his surname was changed but I have only been able to find his birth certificate in his original name. Would he have used his birth name or adopted name to enlist? He married under his adoptive name after the war but I cannot find any record of him during the war under either surname.
Hoping someone can help

WW2 records are still with the MOD. You can order a copy for £30 but they could take a number of months to arrive.

Exactly the same thing happened to my father in the late 1920s. The lady that we always thought of as his big sister turned out to be his mother, and he was brought up by his grandmother. They both thought they'd taken the secret to the grave, but the Internet has subsequently proven otherwise! My dad managed to start school, do national service, work for 40 years for the same major organisation, draw a pension, without knowing the truth, until he tried applying for a 10 year passport in the mid 1980s.  I've not known this very long myself, but having mentioned names of my ancestors on my website, I was contacted by TWO unknown first cousins, so even at age 60 I still think of his birth mother as 'auntie' and HER mother as granny.  Even one-year passports didn't need much proof 50 years ago.  We had several European holidays.

Thank you for the responses. I have printed off the forms to send to the MOD but I don't know which surname my father used. Will I have to apply and pay twice?

If he was formally adopted after 1926 you would expect him to enlist under his adoptive name. If informally adopted before 1926 he may simply have taken his adoptive parents name, or it could have been changed by Deed Pol, if the latter it should show up in the London Gazette.

In applying for his records from the MOD you can give the name  he was registered at death (you will need to provide  a death certificate if he died less than 25 years ago) and include  alias his birth name.
