Deciphering WW1 Medal Card Frank Roper 5 DG

Hi,
I'm trying to decipher my Great Uncles Medal Card, pretty straight forward except for two entries I could do with a little help.
Under the entry for his 14 Star which he got at Mons is a line I can't decipher its says
'???' then looks like July 15 39+. Any ideas ? We do think he came out of Frontline service due to illness after first Ypres in 1915, would this be something to do with that ?

Second is the entry btm right ending in 5392, what is this ?

I've got the kings regs 392 xxi entry for end of engagement in 1917 so no need to answer that.

Any help much appreciated, pic also attached if that's any help !

Think its C+R issued July 1939 -Clasp and Rose for the 1914 Star.
He was discharged under paragraph 21 as you already have that info.

The other bit is a receipt  note saying Clasp and Rose sent (5392)
