Anyone know what this means? Short Service Form

Hello I was wondering if anyone could tell me if a form we have found mentions the reason why he was discharged?

The form is a short service form for the Lincolnshire regiment written in 1915, and written over the top presumably in 1916 in a crayon type substance is:

A 'W' top right hand corner, then over the middle of the form a large '1916 Discharged - C142'. There is also a very smudged smaller piece of writing near the 'W' that looks like a _II_M_ but we can't read the letters in between?

Any help would be greatly appreciated.

The screenshots aren't readable -  a first stab is that sort of overwriting is something done by somebody filing/checking records at some time and means nothing.  If the record is on line, if you give the number and name of the man we can look at the record.
