This may explain things  (from the intro to Roll of Commissioned Officers in the Medical Service of the British Army, 1727-1898):

That is most helpful.  I was reading through the intro when notification of your post pinged my laptop, but had not reached that point.
Thank you and a very Happy Christmas to all who have helped.
Now closing down for a couple of days!

In your post here on Sunday you kindly gave an extract from 'Commissioned Officers...' which is very helpful.  I have managed to see a full copy online but only in transcribed form.    There is a footnote '1' marked in the version you posted which may point to another source document and does not show in the transcription.  Any chance of your sending the footnote also?
Many thanks again.
Ainslie
