Poland: Translation of War Records

I am helping a friend to research her deceased father's WW2 war records.  We have obtained his Army records from the MOD but there are several documents in Polish!  The printing is translatable via "Google Translate" but the handwriting is too difficult, although someone who speaks Polish could probably read & translate it!

Any help or advice would be very much appreciated!
