If you recall I said previously that there had been an error regarded his 15 Star as he qualified for the 14 Star.
This is his Medal card for that.
The jargon says the Medal (15 Star) was returned so it had been received by his widow & the 14 Star had been issued & received.
There's no reason to believe the other 2 hadn't been received.

A wee bit to add to Jim's valuable info.

His arrival date and the medal roll show that he went to France with 1st Battalion R Irish Fus so transferring (fatefully) at some time to 2nd Battalion.  The diary for 1st Bn is at

I have had a quick trawl through both looking for "X men transferred to 2nd....or "X men received from 1st but without success.

The action took place just to the south east of St Eloi.  You can relate the map that jim1 posted to the trench map (which is from later in the War) and the present day map image at this link:

The enemy were coming from the south east.  Trench 19, looking at other maps in other diaries, was somewhere near the present day Eekhofstraat. 

If you want to look at the area on Google Earth for example, take care to search the present day "Sint Elooi Belgium".
