Chronic bronchitis many years. Disease of cardiac valves / ? ? ? ? / unknown Ascites. 4 years

I see chronic bronchitis many years
then disease of cardiac valves ? ? ascites 4 years

The word after Valves is (Semilunar).

Unknown relates to the length of time.
