Does anyone recognise the place of birth noted on the top of this form.

Hello can see the problem you have. In Mayo there are many places starting Bally.
Ok Try going to. www.leitrim-roscommon.com just put in Mayo maybe it will help.

Might it be Ballybuck? I have just googled Ballybuck, Mayo -- and there is such a place.

There is a townland called Bellaburke in the civil parish of Ballintober. However, if you google the words Ballyburke Mayo you will see a number of hits come back for that same place, whether the use of Ballyburke is the result of mistranscription, misspelling, or simply a local naming variant, I don't know.
Here is Bellaburke in OSI's 6" mapping series of 1837 - 1842:

So is it possibly Ballyburke (ie. Bellaburke)?
Edited to add: By way of example, in this record from the Tithe Applotment Books (compiled 1823 - 1837), you can see it shown as "Ballyburke" -

Thanks for all the info, I have looked at Ballybuck but that seems to me to be in Galway , Ballyburke looks very likely , but in another part of the papers I have it states he was born in the parish of castlebar, would that be the county town and ballyburke be within in.
This man is proving to be very elusive !

If you go to Sean Ruad's Irish Atlas on www.thecore.com/seanruad/ and toggle only to Mayo in the county menu, it will bring up the names of all townlands in Mayo in alphabetical order. See what the atlas brings up.

Thank you for that I haven't looked at this for a while as sometimes I think it better to put to one side and look at later with fresh eyes. I am leaning towards ballyburke as suggested previously and is also known as bellaburke, I am not sure if this will give me any more clues to Thomas Butler , but it has certainly revived my quest to carry on searching.
