Can anyone decipher the details on this document, I've avoided posting any personal details about Robert Bonallie (who died in 1926) so hopefully there isn't any copyright issues, any help would be appreciated

Looks like his medals, ribbons and awards for service in The Royal Naval Reserve in WW1. They were sent to the DEMS office in Sunderland. Presumably Mr Smith is the head man there.
I assume this is your man.

His medals are attached - they will also be on the RNR record if you have it. Can you tell me the name of the document you posted please?

Sorry for the delay in replying. Many thanks for your replies, the documents were Form sb36 (the one in my original post, which I downloaded from ancestry when it was free at the weekend) the other ones I have are ADM 171/125 and z/4084 but somehow I had missed the DA 6096 records
