Coronary Occlusion and Stenosis
I think the first line says
This man collapsed & fell (etc as you have it)
Pronounced dead at Y F(?) ..............
(As above)
Sorry, can’t figure out that last bit, is there a hospital or something of a name that looks that shape?

Cause of death
1 Myocardial Degeneration
2. Coronary occlusion
and sclerosis
3.Concussion of Brain
4. Laceration of scalp
Manner of injury This man collapsed o(r) fell down the
stairs of the Vancouver Club & was
pronounced dead at N.V. ??? Hosp(?)
IF it is N.V. (North Vancouver) at that time the hospital was NV General (now called Lions Gate Hospital), but it doesn't say "General". Odd that he would have been taken to North Van anyways. However, at the top of the form, it should say where he was pronounced, above his usual address. It was registered in Vancouver, not North Vancouver, so N.V. might be totally wrong!
Edit:
Here's a link to the Vancouver 1942 directory. I'm not finding anything that would fit, but my eyes aren't what they used to be!
https://bccd.vpl.ca/index.php/browse/title/1942/British_Columbia_and_Yukon_Directory
On the left, select Classified Business Directory, then click the H option (2 above "Hot") to get to the list of hospitals.

My thanks to you both!.
