World War One / Deciphering Medal Rolls Index Cards

Until recently, I believed that my great-grandfathers war medals had been lost when he moved from England to Australia after the war. However, I recently found his Victory Medal and this made me think some of the other medals may not have been lost.

I need some help deciphering the medal rolls index card below, specifically referring to the silver war badge, the writing in the remarks section and the note in correspondence and what medals it might refer to being disposed. Appreciate any help!

World War One / 104th Regt and King's Own England

I have been trying to track my great-granduncle for a long time, with Lancaster being the last place I knew he was. However recently, I found a Canadian war record which seems likely to be him, although I've been unable to confirm it.
In the former service section, it lists "104th Reg. King's Own Eng 6 months", but have been unable to find a Thomas West born approximatly 1893 in Lancashire serving in England before this Canadian service in 1916. Any help with this would be appreciated. I have access to both ancestry and findmypast to look at records. His regimental number in Canada was 790847.

I've never been able to find a photo of my great-grandfather, Joseph Maurice West. However, recently when I was rereading his WWII record, I discovered a note about a photo being taken. Would appreciate any help with finding out if these photos were lost or are still available.
