How did Margaret get to conceive Agnes if she had been admitted to an asylum 2 - 3 years earlier?

I think that is the real question. I really need to pinpoint him. Maybe he was another patient? Or maybe working close by? I'm in the process of trawling the records of the asylum - nothing pointing to any births there yet. But they are vast...
