All my surnames used as middle-names (or in one case as a recurring first name) have been maiden-names. Most have been from the mother of the child but some I have had to trace way back to find where they came from.

It would be worth looking for her in the Poor Law records as well

I agree the transcripts on the Buckinghamshire Marriage Index for the Ann and Elizabeth 1821 marriages both state the brides are widowed - so as you say can't be yours. Also the other marriage you mention, Wlliam Holton to Susanna Norman in 1810 in Buckingham, is not on the index.
How very puzzling that Ciderdrinker is seeing different results ???
