California death cert - QUESTION ABOUT ONLINE ORDERING
« on: Thursday 09 February 06 21:00 GMT (UK) »
Hello all,
I don't usually post in the USA board, but I have one family member who lived and died there and wanted to get hold of her death certificate, if possible.  The lady's name was Maud St Francis INGRAM later SANTANGELO.  She also changed her name to Millicent  :o !

I received the following information from a Rootschatter, from the California death index:

Information removed due to copyright violation. See http://www.rootschat.com/forum/copyright.php for more details

RootsChat must deal with any breach of copyright by its members.

For some time the team of Copyright Editors has been removing breaches of copyright and sending detailed personal messages to the member that had posted the information.  Due to the volume of posts and members this is now impractical.  Messages in breach will simply be deleted and this notice posted.  We apologise for any inconvenience caused but are sure you will appreciate the importance of
this issue.

Can anyone please advise how I could go about getting a death certificate?  I have so far been unable to trace her husband or children, and am hoping it might give me that information.

Any help much appreciated!
Thank you

Hi Sarah,
Thanks for that - I've followed the links and am now in the throes of ordering a certificate, but I've got a hitch:  the form asks for the town and county of death, but the only reference I have is from the death index which shows only one place of death - Fresno.
Would that refer to a city or a county, or both?
APpreciate any guidance here  :)

Cheers

Thanks for that Susan...so do you think I would be safe putting "Fresno" in both the city and county boxes?  They are both mandatory fields on the form, unfortunately  :-\

Yes, that is the way to do it.  There are many towns located in counties of the same name.  For example the city of San Francisco is in San Francisco County.

Re: California death cert - QUESTION ABOUT ONLINE ORDERING
« Reply #7 on: Friday 10 February 06 04:18 GMT (UK) »
OK, I'm back with another question  ???

I've just gone through the ordering process online, pretty easy I thought, and filled in my payment details...then it said to finalise my order I have to print off a form, fill it in and get a Notary Public to sign it, and then fax that to the office, before they will deal with my request.

I have a few problems with this:  1) I live in Australia and we don't have Notary Publics (though I am assuming they are the same thing as our JPs); 2) The form seems to be specifically for people who live in California; 3) I didn't get alerted to this need before I made the order!!

Has anyone else here ordered a US certificate from overseas?  What was your experience?

Thanks
Prue

Prue,

I have not ordered a record from California so I have been looking at various webpages and found one from the Department of Health Services which says that a "CERTIFIED INFORMATIONAL COPY"  does not require a sworn statement about your relationship to the deceased.

It will contain the same information as the regular certified copy but will be stamped "Informational, not a valid document to establish identity"

This link will explain what I have said and can be used to mail a request for a death certificate.

http://www.dhs.ca.gov/publications/forms/pdf/vs112.pdf

Check and see if you have the option to purchase the Certified Informational Copy online.
