Could some kind soul provide the UK address for obtaining the BVRI CD's (and the purchase price) a friend wants to buy them and isn't too keen on buying them from the USA

Here's the answer: LDS Church Distribution Centre, 399 Garretts Green Lane, Garretts Green, Birmingham B33 0UH.  (Fax orders 08700 102052, Credit/debit card orders 08700 102051).  I bought the index in 2002 I think for £22.95. I'm not sure what it would be now.
