Can't read the Manifest

Can someone help me read a ships Manifest please.  I am trying to read the header on the 2nd page of a Manifest from England to New York in 1912
I can't make out point 19 or 22.  There's no point in attaching a copy as the resolution is so bad that the more you magnify the harder it is to read.  I've been back to the Ellis Island site but its no help!

I'm hoping someone may have a more readable copy and that the Manifests don't vary too much!

I believe the ones you are probably looking for are:

19. Ever-in prison or almshouse or institution for care and treatment
    of the insane or supported by charity? If so, which?

and

22. Whether coming by reason of any offer, solicitation, promist, or agreement, express or implied, to labor in the United States.
