what are the headings of the columns?

left one : Name, and Maiden surname of Mother
right one : Signature, qualification, and Residence of Informant it not of the house in which the Birth occured

If you submit a Contact Form to ScotlandsPeople, I'm sure they'll provide a more legible copy!

The informant is Alex Fraser, Father, Not present (i.e. at the birth), who signed with his mark; his mark was witnessed by the Register.

his mark means that he cannot write.He marks the page with an x and the registrar will write the fathers name beside it.Also place of birth should be in the first few columns of the birth cert

I think informant's name is actually "Hy" i.e. Henry  :-\

I think the registrar has the surname Kemp
