Searched for 'Clare Spain' in 1911 census (since she married in 1918 thought she'd be easy to find) and there's this one-
Clare Mary SPAIN age 47 single

If it is her, wonder if Charles might have been married previously.

Thom's 1914 lists a possible connection in the Registered Medical Practitioners In Ireland section :

  C. J. MacAuley, M.B. B.S., N.U.I ., Belfast

I'll see if there are any other details on him..

The one I posted may not by the right Clare or else the children listed are from a previous marriage of Charles'. At the moment it's just a matter of searching to see the possibilites.

found a listing for Charles in 1938 :

  Charles J. MacAuley, M.B. F.R.C.S..Eng, surgeon to Mater Misericordiae Hospital
   63 Fitzwilliam square

   (it's printed in bold in the index, which means he paid extra...)

there's also :

   Henry F. MacAuley, M.Ch.(Belf), F.R.C.S.I., surgeon, 51 Fitzwilliam square
