help with signature plz

hi i am having trouble deciphering signature of applicant and also wht it say's under date and time of deat looks like RW??? any help greatly appreciated...thanks ..

I think it is a signature - R W Munday

Signature I think is S Paul and then Morward.  Difficult to say whether Morward was an address rather than part of the name but I would say it was an address
