How to write a letter in German - COMPLETED

I know I've seen it somewhere but I can't find it now so I'm hoping someone can point me in the right direction.  I wish to write to apply for a marriage certificate but my German is very minimal.

Family Tree Maker (or at least older versions like mine) has some form letter  in the "how to" section. 
However, there's a useful article here:

It says good English is preferable to poor German.
