Hi all

As I live abroad and do not have a Welsh postcode, I am not able to register with the National Library of Wales to use their online electronic database.

Would it be possible for someone with access to the database to retieve the following record for me please? I have no idea what form the it is in, whether it is a transcription or a copy of the original record.
