Hextall would be her maiden name and Smeeton her married one. I can't find a marriage though, could she possibly have been married 3 times?

Best idea - is to send off for Walter and Annies marriage... it may explain a bit more.. I have Walter and Alices cert.  but didnt get his 2nd marriage. 

on to that as of now.

Recieved the marriage cert for Walter and Annie...

So yes her married name was Smeeton aged 33 yrs Widow and her fathers name was Richard Hextall.

the witness are Alfred George Dagley and Nellie Gohsill (?)

The name Dagley is a family name.. so that helps.

Will look into more later  :)

Do you think Richard and Martha Hextall are her parents?

Nellie, father Richard marries Herbert Henry Gopsill 26 October 1912 Earl Shilton

can see a daughter Annie to Richard  till 1901
