Death Cert Formats
« on: Wednesday 08 July 15 22:22 BST (UK) »
Hi

I have two Death Certificates.  One for my Paternal Mother, and the other is for my Paternal aunt.  Now, I am just curious.  On looking at the two Cert.  the formats appear to be totally different.  Does anyone know when (in what year) they were reformatted, so to speak.  As I can see no clue on the form itself.  Hope that makes sense

Both have headings on the Death Certificates of "Extract of an Entry in the Register of Deaths"

Thank you

I think England & Wales birth and death certificates changed to A4 format in 1969

After 1969, death certificates included a date of birth and, for married women, their maiden surname

Well the certificate for my great grandfather - in 1914 - is clearly headed "Certificate of Death" - but that is England.  :o

Oops - sorry - see you are referring to a "short form" certificate  :-X :-[ :-[

Certificates in Wales are now bi-lingual,  but I do not know the commencement date  from English Language only.

Sometimes, the "informant"  may not know details required to accurately  complete all the  required boxes on the form.

Place of birth;  marital status; occupation  and other details may not be known by the "informant"
