Bridget Hobbs death 1934 Portumna

I want to order the death certificate of my ancestor Bridget Hobbs who died in 1934. She was the daughter of Thomas Hobbs and Maria Dignan Hobbs. I was able to find the GRO online application to get the death record but don't have the exact date of death. I am concerned that I will be charged an extra fee for this reason or won't get it. According to records in a genealogy website I do have some information (see below) but there isn't a place to enter this information in the form. Does anyone know if I will be charged or declined because I don't have the exact date of death? Thanks so much for the information!

Here is clipping of the form (there is not blank space to give me the information that I have unless I put it in a field like place of death or former residence.

Thanks for the link. I successfully ordered the death certificate of Bridget Hobbs (after dragging my feet for months). She died in Killimor, 9 Nov 1933. I would like to know if anyone is aware of a newspaper from this area? I would like to try and find an obituary. I am trying to find out some of her other living relatives (Hough being one surname) and where she was buried. Thanks.
