Help needed with recent Death certificate

I've ordered death certificates before but not anything so recent. There seems to be an additional reference required. Is someone able to give me an example of what I should be putting in the reg box at the bottom? I do not know the exact month the person in question died, it doesn't say on the index - or is it referring to dob which obviously is supplied?

Hi yes I have ordered death certificates before no problem with those fields. This is an additional field 'reg' which asks for date and year. Of what I'm not entirely sure. It is obviously after 1960 something when your dob was added to death certificates. I get the feeling this must be what what it is referring to but why call it reg? I can input the month and year of birth as it's in the index (1990 death) but is that what it is asking for?

Hi thanks, I understand what the format looks like I'm referring to the gro website and an additional field that isn't normally there when I have applied for earlier certificates.
