From January 1984 onwards, the GRO death index gives the month of registration, not the quarter. So June in the index (for example) indicates a death registered in the month of June.

Ancestry have generally handled this correctly, but FindMyPast have 'reduced' all the months to quarters, so you can't easily order a post-1983 death certificate from the GRO using FindMyPast's references. You will have to get the month from another version of the index (e.g. Ancestry, FamilySearch, or the GRO's own fiche).

As regards filling in the form, if the death-index entry shows (for example) June 1990, in the box labelled Reg (month and year of registration, mm/yy), you enter 06/90.
