Thank you.
It is strange- the death of their child (I assume it is their child),  John Audley was registered so it seems unlikely that the family would not do this.
However, as you can't find a registration, but you have the memorial card, it seems it is possible that he was not registered.
I wonder if there is a church record of the burial?

Hi Hallmark,
Thanks for your comment however it appears to be inconsistant with the link to the funeral directors question and answers :

Q.   Can a funeral take place pending the issue of a Death Notification Form?
A.   Yes; once it has been verbally confirmed that the doctor is in a position to issue the Death Notification Form, the arrangements for the funeral to take place may proceed.

Yes, the Doctor issues a Death Certificate.... which is not a Registration

The Death Notification Form *

The Death Notification Form is issued by the Medical Doctor/General Practitioner who attended the patient in his/her last illness.

This Doctor must have attended the patient within 28 days prior to death and must be satisfied as to the medical cause of death.

Part 1 of the Form must be signed by the Medical Practitioner stating the cause of death.

Part 2 of the Form must be completed by the next-of-kin/relative giving additional personal details of the deceased, e.g. date of birth, PPS number, occupation, marital status, address, etc, etc.

This Form, having been completed by both the Doctor and the next-of-kin/relative, should then be taken to any Registrar for Births, Marriages & Deaths in Ireland where the death will then be registered.

This must be registered within 90 days of the death taking place.

Deaths should be registered as soon as possible and no later than 3 months from the date of the death.

You will require the written permission of the Registrar General to register any death that was not registered within one year
