Help reading injury record, please

This is a snip from the WW1 Attestation papers of George Michael Hincks (indexed by Ancestry as Hicks).

For 21.4.17 I can read "GSW .....?....  and face       ....? ....

which has been dittoed for 19.4.17
and then again for 9.5.17 but with something extra towards the end of that line.

Can anyone please help me with what it says?

PS : He died of his wounds on 13.5.1917 in Sheffield  :(

18/4/17 from Officer Commanding-wounded.
21/4/17 Admitted to no. 17 Field Ambulance with a gunshot wound to the ? & face.
19/4/17 Admitted to no. 58 Casualty Clearing Station (obviously a mix up in dates).
9/5/17 Admitted to no. 7 (looks like) Canadian General Hospital at Etaples.

Just to clarify he was wounded by shrapnel not a gsw. The RAMC rarely discriminated between the 2.
He was working on a road at Mazingarbe in A Coy. when they were hit by a shell.

Sorry - am easily confused - but war diary extract seems to say that two officers (named) were wounded and two (unnamed) men were killed.  Hincks wasn't an officer, and he wasn't killed outright, so where does he fit in, please?  ;D
