Help with identifying my family's Crest/Coat of Arms

First of all, as I am new to this forum, I want to say hello.

Getting to the point - I am a descendant of a Kelm family, living in Poland, but probably of German/Prussian origin, although I also know from my father, that his father told stories about family roots ultimately reaching to Sweden. Neither I or my father know any more details, besides the unverified information that we come from a noble family.
Sadly, that's basically all I have, and until now I didn't succesfully track my roots deep enough to verify the information.
BUT. There is a page from a heraldry book (dropbox links attached), passed in my family from father to son, depicting a crest/coat of arms. Can anyone please try to help me verify what country does it come from? Is it a real one or a fake? What book is it, that it comes from? Those questions bother me for years, I really hope I can find any track I could follow.
Sorry for my English, it's far from perfect, and if I messed up something with posting rules -sorry again. I would greatly appreciate any help.

It is likely that the name Kelm has itīs origin in the Slavic language area and
could be derived from the Wendish or Old Polish word chelm.
Chelm means hill or slight hill. The Russian word cholm has the same meaning.
In the Indo-Germanic it means to rise, to rise and to stand: hill, slight hill (Latin collis: the hill).
The approximate origin of the name Kelm is in the former German east territories
(east of the Oder-Neisse line).

Dave, thanks as well, I've been digging on genetyka.genealodzy.pl already, but without much success regarding mentioned coat of arms. In the past I also encountered some of the information similar to the ones from the article you quoted, and I'm following this track. Is this publication available online?
