BYW the image you’ve uploaded isn’t in focus, I’ve been looking at another download. Perhaps worth your putting up a clearer copy for others to look at.

Reference the C & A,just had a look at my more modern service record and I don't think it would of changed much over the years,these are what you would find under the Headers:-
 Character            Ability
VERY GOOD                    SUPERIOR (ABOVE AVERAGE)
VERY GOOD*                  SATISFACTORY (AVERAGE)
GOOD                            MODERATE ( BELOW AVERAGE)
FAIR                              INFERIOR (INEFFICIENT)
INDIFFERENT
BAD

As for the FE for age,thats got me stumped.
