I think the name is Andrew Samuel, the informant ,but the residence is so faint.
Hope this is of some use.

I think the next bit might be

Fraternal
uncle
(not present)

but I'm struggling with the last line - presumably Andrew Samuel's address.

It might help if we could see more of the certificate to get a feel for the writing and how well/badly certain letters have copied; or if you could tell us where this is, in case we can get a clue from a map etc.
Researching among others:
Bartle, Bilton, Bingley, Campbell, Craven, Emmott, Harcourt, Hirst, Kellet(t), Kennedy,
Meaburn, Mennile/Meynell, Metcalf(e), Palliser, Robinson, Rutter, Shipley, Stow, Wilkinson

Cause of death Scrofulous
Abscess of the lungs.Of many years.
Scrofula was a form of T. B in the throat glands I feel pretty certain so an abscess in the lung could be a secondary flare up or something not really
connected.
Surgeon was ? Anderson.
Andrew Samuel was his uncle.
Viktoria.

I thought the last word might be something like sliach  -or slioch but Slioch is over the other side in Rossshire.

Have you tried finding Andrew on a census?   it might give his residence.
