For anyone looking for personnel records of a Canadian WWI military personnel, here's the link for the Library and Archives Canada (LAC) database for the First World War personnel records.  LAC has digitized the entire collection of the personnel files.  Not only can you find the attestation papers but there will/should also be a PDF link to the entire file (look for "Digitized service file - PDF format").
