It's a mess
I think the child is a Kelly, possibly James.
The father's name seems to have faded completely, can't make sense of the mother's, looking at others on the page I don't think they have the mother's maiden names.
The name on the end is I think the priest M. Kincella perhaps  I can't even decide if it does begin with a K or not.
Two witnesses underneath.
Sorry I'm not any help.
Hopefully someone else will be.

Date June 24th?
I agree name under blot might be Kelly. I thought forename began with B. Barnabas? Letters further along look like ditto but the letters which I think are t may be l.  :-\
I don't know to whom the name on end of that line belonged. Neither am I sure what it was. Might be Kinsella. Seems to be Ma and possibly other letters before K. MacKindley? Mary Kinsella?  :-\
Sponsors on line below. Forename of 1st sponsor might begin with M. There's a surname at end of that line, is it a sponsor or priest?
I can't see headings of columns. Seems to be no pattern to format of entries, although as I could read only a fraction of each, I can't really judge. Have you looked at other pages in case they are more legible, enabling you to make sense of format?

Edit. Reading a few pages on, where I can make out an occasional almost complete, almost legible entry, I think Sinan is correct that mother's maiden names weren't included. I couldn't see priest's name either, or townland. If the surname at end of first line was that of mother, perhaps parents weren't married.
Examining surname under blot again, it may begin with R. (Riley/Reily?).

Daniel,
Are you transcribing a register or have you searched a name and this was the record.

Thank you to everyone for your replies.

The handwriting is certainly "very challenging".  I'm researching the family of Michael and Catherine Kindelan.  The names on the left, I feel are the sponsors of the baptism.  Hope this helps.  Thank you Daniel.
