Help with cause of death please.

Please can anyone help me with the Cause of death. I can read some but words but not others.
It starts Disease of the heart. Some time.

Then I can guess at some but need help please.

Did the death take place in Canonbie?

If so, transcription from the 1861 census, Eskbank, Canonbie.

Aennes Macaulay, Surgeon and Physician aged 38, born Caithness.

Only listed surgeon I can find for the time with surname Macaulay is a James Macaulay but he became involved in literature so doesn't look good, his father was a physician though and named Alexander Macauley 1783–1868.
