help translating antwerp immigration doc

Can anyone help me by translating the handwritten answers on this document?

I'm sorry its a photograph of a computer screen, Ive tried uploading the documents but it wont let me as it says the documents are too large

After you have resized an image to 500 kb or less, you will have to rename it before it can be uploaded for a second time.

Just to add that the name must be unique within RootsChat ;D
I usually add -KG to the filename! (My initials)
